The document is available to view here and is provided to offer assistance in completing your RFU Registration.               RFU Player Registration Form This is the Adult Player Registration Form. Please download here and print off a copy. Once you have completed it then pass to Mo Byers who will ensure you are registered with the RFU.               Blandford RFC Membership Application This is the Application Form to become a member of Blandford RFC and is available here. Details on how to submit your membership are included with the form.               Blandford RFC Accident/Injury Occurrence Report Form This is the Accident/Injury Occurrence Report that should be submitted to the Honorary Secretary in the event of an injury.               RFU Reportable Injury Event Report Form This is the Report Form that should be submitted to the RFU following any injury that results in a player being admitted to hospital.
